PeaceHealth Southwest has teamed up with Relay Health to bring the SmartConnect program to the southwest Washington community. SmartConnect will be beneficial to physicians and patients because it will be a one-stop shop for patient health information such as lab results, imaging reports, and emergency department visit discharge summaries. No more getting on several different portals or having to pick up the phone to hunt for information about a patient. When a patient says, "I remember having lab work done, but I can't be sure of when or where," providers and patients will have easy access to that information.
SmartPath Clinical Documentation (Electronic Medical Record)
Provides the ability to document patient history, physical, and treatment plans, share records across the care network, increase collaboration and increase patient satisfaction. This simple electronic medical record was one of the first electronic health records in the nation to be certified.
SmartPath-Clinical Reporting
Create practice- and provider-level scorecards to allow providers and practice administrators determine the status of compliance with PQRI and/or Meaningful Use measures, analyze individual provider performance and compare previous measure years.
SmartPath-Bidirectional Orders/Results
Lab and Radiology orders will be electronically ordered from your EMR or directly in the website and results will be returned to your clinic. Clinics can determine whether results should be released to the patient’s message box and when this should occur. This frees up staff from printing and mailing results and ensures that delivery to your clinic occurs in real time.
SmartPath ePrescribing
Electronic prescriptions with pharmacies are readily available and quickly done with this module. No more looking for the prescription pad.
SmartPath-Messaging (Secure email, appointment requests, colleague referrals)
If you want to securely communicate with patients, allow them to request appointments on line or quickly refer to colleagues and see the status of the referral, use this secure messaging module. Clinics have control over which functions they allow. Prescription refills, patient results as well as direct messages to the patient are all an option with this module.
