You are entitled to receive copies of your record. To protect your privacy, you will be required to provide proof of identity. There may be fees. To obtain a copy of your records:
Certified birth or death certificates are issued by the State of Washington. You may order them online at http://www.doh.wa.gov/ehsphl/chs/cert.htm or call (800) 525-0127. The mailing address is Washington State Department of Health, 101 Israel Road SE, Tumwater, WA 98501.
