PeaceHealth Epic Project

Planning is well underway to transition all of the PeaceHealth facilities to Epic's electronic health record. The Epic Ambulatory Project for PeaceHealth Medical Group (PHMG) will see the first clinic go-live in May at Peace Harbor in Florence, OR. PHMG practices at PeaceHealth Southwest (PHSW) will go-live in early 2014. The business plan for the Enterprise Project will be completed in March Once approved by the PeaceHealth Executive Team (PHET) and the PeaceHealth Board, the goal is to have all facilities transition to the new Epic system by fiscal year 2017.

ICD-10 and Provider Documentation

On October 1, 2014, all healthcare entities in the U.S. will be required to submit billing information using ICD-10 instead of ICD-9 codes. In preparation, some of the electronic note templates are being revised to provide guidance for more specific documentation. Provider representatives are working with Clinical Documentation Specialists on the note template revisions. The new and revised templates are being released over the next several months and communications are being sent to the specialty groups who are affected.

Tips & Tricks

EKGs/Muse/CV Web – In addition to viewing EKGs from the EKG tab or Patient Folder, you use the CV Web icon in Application Portal directly. This allows you to look at EKGs without having to log into Physician Portal. Detailed information is available at this link.

Open (Unsigned) Notes – There are still open (unsigned notes) attributed to providers in Expert Notes that need to be signed or removed. HIS (Medical Records) has been assigning these to your deficiency list. Detailed instructions for removing unsigned notes that need to be removed are available at this link.

Change Requests - Requests for changes to orders, outlines, iForms and note template can be submitted on the PHSW Intranet -> Everything Computer -> Clinical Content Change Request. This is the quickest and most efficient method for requesting changes to clinical content.

Blood Transfusion Orders

Blood and blood related lab tests, such as a Type and Screen continue to be one of the most challenging and error prone order processes in CPOE. All orders for a Type and Screen need to be done through the Transfusion iForm. Clinical decision support and duplicate checking is built into the form. Newer evidence supports the transfusion of one unit of PRBCs at a time for most indications. Significant work is underway to re-design the iForm to help prevent duplicate orders and to provide clear instructions to facilitate provider workflow.

PeaceHealth Southwest Cancer Center Honored with Accreditation and Commendation

PeaceHeath Southwest Medical Center is pleased to announce its Regional Cancer Center has received a three-year Accreditation with Commendation from the Commission on Cancer (CoC) of the American College of Surgeons.

A facility receives a three-year Accreditation with Commendation following an onsite evaluation during which the facility demonstrates a Commendation level of compliance with one or more standards that represent the full scope of the cancer program. In addition, a facility receives a compliance rating for all other standards.

PHSW Regional Cancer Center received a perfect score for its compliance rating, as well as all eight commendations. A commendation is awarded when an organization exceeds the standard set by the CoC. Read on.

Labyrinth Offers Contemplative, Healing Journey

   
Cultures and religions throughout history have recognized the sacredness of life as a journey. The labyrinth is a metaphor for our journey through life, reminding us that life is a process in which we seek healing and wholeness. Labyrinths date back at least 4,000 years and are found in many religious traditions around the world as a way to physically represent life's journey.

PeaceHealth Southwest's Spiritual Care Department will sponsor a Labyrinth event for caregivers, patients and the community on Wednesday, March 20, 2013, at the PHSW HEC Auditorium. Read on.

A labyrinth is a one-way path that spirals or winds inside a circular shape. Walking the path leads to the center with the same path being followed back out. It is not a maze. The labyrinth serves as an image for one's walk with God or one's journey through life.

Walk the Labyrinth

What: PeaceHealth Southwest's Spiritual Care Department will offer an indoor labyrinth for walking and contemplation by caregivers, patient and the public.

When: Wednesday, March 20, 2013, 12:30 p.m. to 4:30 p.m.

Where: PeaceHealth Southwest Medical Center, Health Education Center Auditorium.

Why: Walking the labyrinth is a form of quiet meditation, contemplative prayer, spiritual pilgrimage and reflection. The experience may also serve as a means of celebrating life transitions. Ultimately, walking the labyrinth is healing.

PeaceHealth Southwest Foundation Honors Physicians and Raises Funds for PHMG Programs

National Doctor's Day is March 30, 2013. Doctor Day postcards will be sent to 5,000 PeaceHealth Medical Group (PHMG) outpatients in early March 2012 and available in PHMG patient waiting areas. The postcards invite PHMG patients to honor their physicians as well as to make tax-deductible contributions in support of the work they do. Read on.

"This effort will not only spotlight our excellent physicians," said Foundation Director Jean Rahn, "it will raise valuable funds for PHMG programs and help up develop relationships with patients who are grateful for their care and who want to contribute to excellence in local medicine."

Funds raised will benefit the PHMG Compassionate Care Fund which provides for a broad range of health care needs for PHMG patients who are uninsured or underinsured. "We're thankful to partner with our Foundation in this way," said Greg Orr, Regional VP of Physician Services for PHMG. "Philanthropy is becoming increasingly vital to providing the best care, with the best equipment and in the best facilities for our caregivers and patients."

Respecting Choices, Internationally Recognized Advanced Care Planning Program

Hospice Southwest and Bridges Palliative Care are proud to offer providers education in Respecting Choices, an advanced care planning program developed by Gunderson Lutheran Medical Center in La Crosse, Wisconsin.

Respecting Choices is an internationally recognized, evidence-based advance care planning program. Since 2000, Respecting Choices has assisted organizations in replicating the Respecting Choices model through the design of key elements. Respecting Choices focuses on the process of advanced care planning that result in honoring health care decisions. Read on.

Heart Connections Program for Cardiac Patients

   
Hospice Southwest is pleased to offer the new Heart Connections Program for patients to decrease cardiac readmissions to the hospital.

The Heart Connections Program has trained the team in the complexities of cardiac disease progression, medications, and symptom management. The team is available around the clock to meet the needs of these patients and families and can better address the end-of-life care for cardiac patients by using more aggressive palliative care, closer monitoring with daily weights, sodium restricted diets, IV diuretics, and lab at home.
