Medication reconciliation is an effective process that helps reduce errors and harm associated with inaccurate medication information.

Simply put, medication reconciliation means you provide the most accurate complete and current list of your medications when you are admitted, we update the list during the course of your care, and we communicate this list to the next person or facility who continues your care.
