What are the benefits of an anticoagulation service?
The PeaceHealth Southwest Medical Center Anticoagulation Clinic is run by pharmacists who are clinically trained to monitor warfarin (Coumadin®) therapy. Studies have shown that anticoagulation management by pharmacists may reduce your risk for bleeding, reduce your likelihood of forming clots, and increase the amount of time that your blood tests (INR) are in the appropriate range. |

MEDICATION MONITORING
 What is warfarin (Coumadin®)?
Warfarin (Coumadin®) is a medication used to prevent blood clots from forming or growing larger. It works by stopping the formation of substances that cause clots. It is often given to patients who have had blood clots or are at risk for developing a blood clot.
Why do I need frequent monitoring if I’m on warfarin (Coumadin®)?
Patients must take an appropriate dose of warfarin (Coumadin®) in order to prevent clot formation or bleeding. Since many medications, foods, and disease states can affect your response to warfarin (Coumadin®), frequent blood tests and discussions with your doctor or pharmacist are required.
REFERRAL PROCESS
Do I need a referral from my doctor?
If your doctor believes this service is right for you, he/she will refer you to our Anticoagulation Clinic. Your doctor must refer you to our service in order to participate. After receiving your referral authorization, you will be scheduled for your first visit.
CLINIC VISITS
 What is involved in the clinic visits?
Our clinic is staffed by pharmacists with advanced clinical training in anticoagulation management. During your visit to the clinic, you can expect the following:
- Each visit takes approximately 15-20 minutes.
- The pharmacist will discuss the use of warfarin (Coumadin®) and answer any of your questions.
- The pharmacist will obtain a small blood sample from your finger. This sample of blood will be used to determine how long it takes for your blood to clot (your INR result).
- The pharmacist will monitor you for any drug and diet interactions or side effects.
- The pharmacist will make adjustments to your warfarin (Coumadin®) dose if needed.
- A follow-up appointment will be scheduled.
Will my insurance be billed for the clinic visits?
Your insurance may be billed automatically. You may be responsible for a co-payment or any unmet deductibles.
